Connecting Ontario

ConnectingOntario is a secure, provincial electronic health record (EHR) system that allows authorized members of a patient’s healthcare team to access patients’ personal health information contributed by participating ConnectingOntario healthcare organizations. Markham Stouffville Hospital is a contributing organization as of February 2018 and will provide its health care providers with access to the Provincial EHR through ConnectingOntario. Information contained in in the platform includes: 

  •  lab results
  • diagnostic imaging reports
  • discharge summaries
  • visit notes
  • certain publicly funded drug information

Access to ConnectingOntario is restricted to those who provide or assist with patient care. The program is funded and overseen by eHealth Ontario.

What are the benefits?
  • ConnectingOntario is the platform that is used to enable authorized members of a patient’s health team to share medical data in a safe, secure, digital format that will improve the care patients receive. 
  • Delivers information to the right people at the right time, enhancing the patient/provider experience
    • Reduces costs associated with managing paper records and duplicate tests and allows a more efficient transfer of accurate information
  • Provides a more complete picture of patient health information across the continuum of care
  • It avoids the need for patients to repeat their medical history when they go to different health care providers.
  • Reduces wait times for patients.
Consent Directives
