If you have confirmed cerebral vascular disease, the Lifestyle Education and Exercise Program (LEEP) at Markham Stouffville Hospital can help empower you to make the best decisions for a healthier lifestyle. You will benefit from this service if you have had a stroke or transient ischemic attack (TIA). This program can also be helpful if you smoke or have any of the following:

  • High cholesterol
  • High blood pressure
  • Obesity
  • Diabetes
  • Increased stress level
  • Inactive lifestyle

Goals of the LEEP Program

The program hopes to achieve the following goals for everyone who actively participates:

  • Improvement in cardiovascular fitness
  • Overall increased feeling of well being Improved blood pressure
  • Improved blood sugar control
  • Improved cholesterol and triglyceride levels

What to expect in the LEEP Program

As part of the six-month treatment plan, you will be encouraged to participate in the following:

Orientation session – This is an introduction to the services provided by our physiotherapist and other members of the team.

Introductory assessment – This is done to evaluate your risk factors. We will also create a personal care plan for you to assist you in making lifestyle behaviour changes.

Educational sessions – During the first three months of the program, you will participate in weekly education sessions on topics such as stress management, healthy eating and medication management.

Exercise sessions – After you complete a personalized exercise prescription from our physiotherapists, you will attend weekly education and exercise sessions over a three-month period. Once that is completed, you will engage in a home-based exercise program with telephone support provided by our physiotherapists. After you graduate from the program, we will connect you with experts in the community who specialize in long-term lifestyle choices to best manage cerebrovascular disease. We will also put you in touch with ongoing exercise venues.

Stress testing – We will conduct physician-supervised, graded exercise tests at regular intervals. These are important tools for evaluating your progress.
