The Heart Function Clinic is a specialty outpatient clinic for patients who were admitted to hospital with congestive heart failure due to a weakened or a stiffened heart muscle. The clinic is staffed by specialty registered nurses, cardiac nurse practitioners, and cardiologists.

The primary goal of the Heart Function Clinic is to help patients and their family members learn about heart failure, lifestyle modifications, and the best available medications so patients can live as symptom-free as possible.

This clinic provides specialty education, close professional support and monitoring, as well as ongoing follow-up, in order to reduce emergency department visits and hospital readmissions for heart failure symptoms.

ViMos is an innovative, patient-centered self-management program that provides remote monitoring and clinical support for patients with chronic conditions, such as chronic obstructive pulmonary disease (COPD) and congestive heart failure.
