Adult Diabetes Education Centre

Markham Stouffville Hospital offers adult diabetes services at four different clinic locations across York and Durham regions. Adults who are 18 years old and older who are at risk of developing diabetes, who have prediabetes, type 1 or 2 diabetes or gestational diabetes are welcome to access our services.

Our diabetes care team includes endocrinologists, registered nurse certified diabetes educators (CDE) and registered dietitian CDEs. Our care team can help you and your family learn about how to manage and live well with diabetes.

All of our program sites offer:

  • Nutrition, activity and diabetes risk management and education
  • Medication education, adjustment and treatment options
  • Instruction on blood glucose monitoring
  • Individual and group education
  • Ongoing diabetes follow-up care

Group information sessions about diabetes

Group information sessions are offered on topics of interest including the following:

  • Healthy eating, grocery store tours, active living and exercise, weight loss management
  • Blood sugar monitoring, risk reduction and diabetes medication management

Referral and appointment information 

A physician referral is necessary to be seen by an endocrinologist. All other appointment types can be accessed by self-referral.

Markham Stouffville Hospital, Main Site – 379 Church Street Markham, ON, room 308

This site offers all Adult Diabetes Program services and is open five days a week:
Mondays: 8:00 a.m. – 4:00 p.m.
Tuesdays: 8:00 a.m. – 9:00 p.m.
Wednesdays: 8:00 a.m. – 8:00 p.m.
Thursdays: 8:00 a.m. – 9:00 p.m.
Fridays: 8:00 a.m. – 4:00 p.m.

Uxbridge Diabetes Education Clinic – 2 Campbell Drive, Uxbridge, ON, room 208

Clinics are run three days per week:
Wednesdays: 8:30 a.m. – 9:00 p.m. 
Thursdays and Fridays: 8:30 a.m. – 4:30 p.m.

Parkview Clinic – 12184 Ninth Line, Whitchurch-Stouffville, ON

A diabetes education team (registered nurse CDE/registered dietitian CDE) runs a mobile clinic every two months for residents with diabetes who reside at the Parkview Village Seniors’ Apartment Building

Aaniin Community Centre Clinics, 5664 14th Ave., first floor, Markham, ON

Tamil, Punjabi, Hindi, Urdu and Gujarati Diabetes Clinic:  

This clinic is run Wednesdays 10:00 a.m. – 6:00 p.m. and Fridays 8:00 a.m. – 4:00 p.m.
Publicly-accessible group education classes are also offered at this site. 

Cantonese and Mandarin Diabetes Clinic:

This clinic is run Tuesdays 9:00 a.m. – 5:00 p.m. and Thursdays 1:00 p.m. – 9:00 p.m.
