Requesting your health information - Uxbridge site

As of July 9, 2018, all requests for medical records, including records from the Uxbridge Cottage Hospital, will be processed at Markham Stouffville Hospital’s Health Information Department. The release of information staff are available from Monday to Friday 8 a.m - 5 p.m. to help answer any questions and guide you through the process of requesting and receiving your records.

If you require a CD of your diagnostic image, please continue to request the CD’s from the Diagnostic Imaging Department at Uxbridge.

 How can I request my records?

If you require a copy of your medical records the Consent to Release Personal Health Information form must be completed.  The consent form can be dropped off, faxed, or mailed to the Health Information Department.

Once your records are ready, we will give you a call and let you know that they are ready for pick up and inform you of the fee amount. When picking up your records, we are required to see one piece of government photo ID.

 Where do I pick up my records?

You will need to pick up your records at the Markham Stouffville Hospital location in the Health Information Department (Room A1681).

 Is there a fee for my records?

If a patient requires a copy of their personal health information, they pay an administrative fee of $30 for the first 20 pages and $0.25 per hard copy page or $0.50 per microfiche page thereafter. 

Records released to a Physician are released to them at no cost. 

 Can my records be released to a physician?

After each visit at MSH provided that your family physician is listed in your electronic health record, they will receive copies of key reports such as laboratory, Diagnostic Imaging, and dictated notes.

If you require additional copies to be sent to your physician’s office, you may complete a Consent to Release Personal Health Information form to authorize release of your records to a physician at your direction.
