How to Access Online Forms and Requisitions

Use these steps to access the online library of Markham Stouffville Hospital referral forms and requisitons.

1) Visit www.msh.on.ca/user

2) Type in the username and password that has been given to you and click Log In

3) Visit www.msh.on.ca/referral (or alternatively find it under the physicians tab)
