Complex Care

We offer Complex Care services to adults who have multi-system healthcare needs which cannot be met in the community or at a long-term care facility. Complex Care patients frequently have acute disabling episodes in addition to their chronic illnesses, and require interdisciplinary intervention. These patients have diagnoses such as unstable congestive heart failure (CHF), severe cerebral vascular accident (CVA), acquired brain injury (ABI), Alzheimer's or Parkinson's disease. The expected length of stay may be months to years.  If healthcare needs stabilize, a return to home or discharge to long-term care may be appropriate.  Our interdisciplinary healthcare team has expertise in wound management, dementia, chronic illness and pain control, and our services are designed to maximize health and quality of life.
