Requesting Your Health Record

To request your Health Record, simply complete and sign the appropriate authorization form, provide payment (if applicable) and submit to the Health Records Department. We process all requests for Health Records in a timely and diligent manner. Sometimes, delays do occur, and we greatly appreciate your patience. We can fast-track fee-paying records for immediate release for an additional $300 charge. Fast-tracked records can be ready for release wtihin two business days.
